The Assessments tool gives your organization a structured way to gather feedback from team members. Owners, Admins, and Coaches can launch assessments, track who has completed them, and review results with downloadable reports. Managers and Team Members complete assessments assigned to them and can view their own results. Observers have view-only access to assigned assessments.
Every Ninety account includes a set of default assessment templates, including the Baseline Assessment and the Org Fitness Review. Organizations on paid plans can also build and deploy custom assessments to gather targeted feedback on topics like team health, engagement, onboarding, and more.
What you see on the Assessments page

The Assessments page is divided into sections based on your user role.
At the top of the page, three controls help you find assessments quickly: a Status filter dropdown, a Browse templates button, and a Search Assessments field.
Below these controls, the page displays one or two sections depending on your permissions:
- Assessments and Results (Owners, Admins, and Coaches only): A list of all assessments launched at your organization, past, present, and future. Each row shows the assessment's title, status, launch date, end date, and owner. Two buttons appear at the top right of this section: Launch existing and Create new assessment.
- Assigned Assessments (all roles): A list of all assessments assigned to you. Each row shows the assessment's title, due date, and completion status. A Start button appears next to assessments you haven't completed yet.
Managers and Team Members see only the Assigned Assessments section. This is the only section available to these roles in the Assessments tool.
Note: Your company may use a different name for this tool. Owners and Admins can customize tool names in Company Settings.
Default assessment templates
Ninety includes three default assessment templates. These templates are available in the Launch existing dialog and cannot be edited.
- Baseline Assessment (No BOS Coach): A one-time assessment designed for organizations implementing a business operating system (BOS) without the guidance of an external coach. Use this assessment to establish your organization's starting point.
- Baseline Assessment (We Have a BOS Coach): A one-time assessment designed for organizations working with a BOS coach. This version includes additional questions relevant to the coaching relationship.
- Org Fitness Review: A recurring assessment you can deploy quarterly (or on any cadence) to track your organization's progress over time. Use the Org Fitness Review after you've completed the Baseline Assessment to measure how your organization is improving.
The Baseline Assessment can only be launched once. After that, use the Org Fitness Review for ongoing measurement.
Launching an existing assessment

Owners, Admins, and Coaches can launch an assessment from an existing template. To launch an existing assessment:
- Click Assessments from the main navigation menu.
- Click Launch existing in the Assessments and Results section.
- Select a template from the Which Assessment template do you want to launch? dropdown. This list includes default assessments (Baseline Assessment and Org Fitness Review) and any custom assessments your organization has created.
- Optionally, enter a name in the Would you like to give this Assessment a new name? field to identify this specific round of the assessment (for example, "Org Fitness Review Q2, 2026").
- Set the Launch date. This is the date the assessment becomes available to the assigned teams.
- Set the End date. This is the last day the assessment can be completed. The assessment closes at 11:59 p.m. CST on this date.
- Optionally, add a Description. Anyone assigned to the assessment can read this text before they begin.
- Select the teams that should receive the assessment from the Assessment template recipients dropdown. This list includes all non-private teams in your organization, plus any private teams you belong to.
- Optionally, select the Create To-Dos for assessment template recipients checkbox. This creates a To-Do for each person assigned to the assessment as a reminder to complete it.
- Optionally, select users from the Send results to dropdown. These individuals receive access to the assessment's results when available.
- Click Launch.
After you launch the assessment, it appears in the Assessments and Results section with an "In progress" status. Each person on the selected teams sees the assessment in their Assigned Assessments section.
Taking an assigned assessment

All licensed users can complete assessments assigned to them from the Assigned Assessments section. To take an assigned assessment:
- Click Assessments from the main navigation menu.
- Find the assessment in the Assigned Assessments section.
- Click Start to begin the assessment.
- Answer each question. If you need to stop partway through, you can save and quit. A progress circle on the Assigned Assessments page shows how far along you are.
- You can go back and change your answers at any time before you finish.
- After answering all questions, submit the assessment.
Once you complete and submit the assessment, it cannot be reopened, and there is nothing more for you to do. The person who launched the assessment receives the results after the assessment closes.
Each assigned assessment also has an ellipsis menu (...) with two actions: Create Issue and Create To-Do. Use these to log follow-up items related to the assessment.
Tracking assessment progress
Owners, Admins, and Coaches can monitor assessment progress from the Assessments and Results section. The assessment's status updates automatically as responses come in:
- In progress: The assessment has been launched, but no one has submitted their responses yet.
- Active - Responses coming in: At least one assignee has completed the assessment, and the end date has not passed.
To see who has completed the assessment and who hasn't:
- Click the assessment's title in the Assessments and Results section to open the details panel.
- Scroll down to the Assessment Status section. Each assignee is listed with their current status (Not started or Completed).
From the details panel, you can also edit the assessment's name, launch date, end date, or description while the assessment is still active.
Ending an assessment

Assessments end automatically at 11:59 p.m. CST on the end date you set when launching. You can also end an assessment early if you've collected enough responses or need to close it ahead of schedule.
To end an assessment early:
- Click the assessment's title in the Assessments and Results section to open the details panel.
- Click End early.
- A confirmation dialog appears: "Are you sure you want to end this assessment early? Any incomplete assessments will be omitted from the results."
- Click End Assessment to confirm.
You can also end an assessment early by clicking the ellipsis menu (...) on the assessment row and selecting End early.
After the assessment ends (manually or by the scheduled date), the status changes to one of the following:
- Closed: All assignees completed the assessment before it ended.
- Closed - Partial responses: Some assignees did not complete the assessment before it ended. Only completed responses are included in the results.
To reopen a closed assessment, click the ellipsis menu on the assessment row (or right-click the row) and select Reopen. This allows remaining assignees to finish their responses.
Viewing results and downloading reports
After an assessment ends, Owners, Admins, and Coaches can review the results and download a report. To view results:
- Click the closed assessment's title in the Assessments and Results section to open the results panel.
- The results panel displays:
- An overall score shown as a gauge chart.
- A Component Scores bar chart that breaks down performance by category.
- An Assessment Status section listing each assignee and whether they completed the assessment.
If some assignees did not complete the assessment, a banner at the top of the results panel notes that the generated reports are not representative of all participants.
To download a PDF report:
- Click Print at the bottom of the results panel, or click the ellipsis menu on the assessment row and select Print.
- A dialog confirms "Your report has generated."
- Click Print to download the PDF.
The Org Fitness Review generates a personalized report that includes your overall score, detailed scores for each competency area, a strength and dispersion chart for each question, and actionable recommendations for improving each area.
Additional actions from the ellipsis menu
The ellipsis menu (...) provides quick actions depending on the assessment's location and status.
On an assessment in the Assessments and Results section (Owners, Admins, and Implementers):
- End early: Close the assessment before the scheduled end date.
- Create Issue: Create an Issue in Ninety related to this assessment.
- Create To-Do: Create a To-Do in Ninety related to this assessment.
- Delete: Permanently remove the assessment. This action cannot be undone.
On an assessment in the Assigned Assessments section (all roles):
- Create Issue: Create an Issue in Ninety related to this assessment.
- Create To-Do: Create a To-Do in Ninety related to this assessment.
There is no archive for assessments. Completed assessments remain in the Assessments and Results list for ongoing reference.
Troubleshooting
Issue: I don't see the Launch existing or Create new assessment buttons. Solution: These buttons appear only in the Assessments and Results section, which is visible to Owners, Admins, and Coaches. If your role is Manager, Team Member, or Observer, you won't see this section or its buttons. To check your role, go to the Directory and find your name in the user list. If you need to launch assessments, ask an Owner or Admin to update your role or launch the assessment on your behalf.
Issue: I want to send an assessment to a single person, not the entire team. Solution: Assessments are deployed at the team level. You cannot send an assessment directly to a single individual. If you need only certain people to take an assessment, create a new team that includes them, then launch the assessment to that team. You also cannot send an assessment to someone who does not have a Ninety account; all recipients must be licensed users assigned to a team.
Issue: I completed an assessment and want to change an answer. Solution: Once you submit an assessment, it cannot be reopened from your side. You can go back and change your answers at any time before you click the final submit button, but once submitted, the assessment is locked. Contact the Owner, Admin, or Coach who launched the assessment if you need to redo it. They may be able to delete the current round and relaunch a new one.
Issue: The assessment status says "Closed - Partial responses." Solution: Some assignees did not complete the assessment before it ended. The results reflect only completed responses. You can still view results and download a report for a partially completed assessment. The person who launched the assessment can reopen it by clicking the ellipsis menu on the assessment row and selecting Reopen. This allows remaining assignees to finish.
Issue: Where do I find my assessment results? Solution: Assessment results are available in the Assessments and Results section after the assessment ends (or is ended early). Click the closed assessment's title to open the results panel, which displays the overall score, component scores, and completion status for each assignee. Click Print to download a PDF report. If the assessment is still in progress, the results are not yet available. You can end the assessment early from the ellipsis menu if you've collected enough responses.
Issue: I can't see individual written responses from a custom assessment. Solution: The Assessments tool shows aggregated results (overall scores and patterns), not individual written responses to each question. Once a custom assessment is completed, the specific text each person entered is not viewable in the results panel. If you need to collect responses for individual review, consider using the description field to let participants know they should also document their answers elsewhere, or use a different tool for open-ended feedback collection.
Issue: Can I remove a person from an assessment after it has launched? Solution: No. Once an assessment is launched, it is deployed to the selected teams, and there is no option to remove a specific individual. If someone cannot complete the assessment (for example, they are on leave), you can end the assessment early. The results will reflect only the responses that were submitted. Alternatively, if you need to exclude that person's team entirely, you would need to delete the assessment and relaunch it to different teams.
Issue: I can't find a past assessment. Solution: There is no separate archive for assessments. All launched assessments appear in the Assessments and Results section. Use the Status filter at the top of the Assessments page to narrow your view, or use the Search Assessments field to search by name. If you still can't find the assessment, it may have been deleted. Deleted assessments are permanently removed and cannot be recovered. If you need further assistance, contact support.
Issue: Can I preview an assessment's questions before launching it? Solution: To preview the questions in a default assessment template, click Browse templates at the top of the Assessments page and select the template you want to review. For a custom assessment you've created, click the assessment title in the Assessments and Results section to open it and review the questions. If you want to test the full experience before sending it to your team, create a small test team with just yourself, launch the assessment to that team, and take it yourself.
Issue: Can I relaunch the Baseline Assessment? Solution: No. The Baseline Assessment (both versions) can only be launched once per organization. It is designed to establish your starting point. After the Baseline Assessment is complete, use the Org Fitness Review on a recurring basis (quarterly is recommended) to track your organization's progress over time.
